As a Home Health Registered Nurse with Well Care Health, you will use an individualized patient care plan to provide top quality care to all our patients, keeping them Happy, Healthy and at Home.
RN Case Manager schedule Full-Time, Monday- Friday 8am-5pm w/minimal on-call requirements No weekends Travel throughout Orange County, NC As an RN Case Manager, you will Visit 3-6 patients on any given shift, driving to the patient's home providing skilled nursing services.
Primarily responsible for the initial assessment and case development of patients in all stages of life in their homes.
Oversee assessing, interpreting, planning, implementing, documenting, and evaluating patients according to the patient's age and diagnosis.
Must be able to check your email intermittently during the week to respond to documentation requests.
Must be okay with traveling throughout the service area! Skills/Experience needed Home Health experience strongly preferred.
Homecare Homebase and Oasis experience is highly preferred.
Admissions, Wound Care (with vac), IV Therapy, PICC Lines, Lab Draws, and Patient Education.
Strong assessment, skilled nursing, and documentation We are 100% EMR and clinicians document in the patients' homes with company-issued laptops.
