Healthcare Ambassador, Lead

The Ambassador is a pivotal member of the Landmark Interdisciplinary Team. As an Ambassador, you will actively engage with patients both in the home and over the phone. You will use your compassion & empathy to earn patients' and caregivers' trust, your persistence to support and advocate for them, and your communication skills to build impactful relationships. More specifically, as an ambassador lead you will:
- Support the administration of the market's Ambassador program, including but not limited to:
o Monitor reports to ensure patients and the interdisciplinary care team receive timely support o Review incoming requests for support and ensure they are assigned appropriately for even distribution of work o Support training efforts and serve as a subject matter expert on the Ambassador role o Support weekly interdisciplinary team meetings, including agenda preparation and follow-ups - Conduct home visits and phone/ televideo calls to patients to collect clinical and psychosocial information, and carry out designated tasks, including but not limited to:
o Resource referral and connection o Quality measure documentation, support, and follow-up o Telemedicine facilitation / set up o Using pre-designed checklists and templates to collect information regarding patient conditions - Document collected information in appropriate systems - Partner with Landmark interdisciplinary team (providers, nurses, social workers, etc.) to drive improvement in patient outcomes and satisfaction - Actively participate in weekly interdisciplinary team meetings - 5-7
years of experience in any of the followings job categories:
Social Worker; Home Health Aide; Certified Nursing Assistant; Certified Medical Assistant; Medication Aide; Health Coach; Health Navigator; Healthcare Technician. Candidates with experience similar to those listed here are encouraged to apply as well. - Exceptional interpersonal skills. Superb written and oral communication skills, and comfort level with technology. - Demonstrated leadership, mentorship and teaching capability - Comfort with conducting independent patient home visits. - Access to reliable transportation to conduct patient visits required; if you are driving a vehicle, you must comply with all the terms of the Landmark Motor Vehicle Safety policy. - Strong preference given to candidates with geriatric or primary care experience, or experience in institutional settings of care such as long-term care/skilled nursing facilities. - Competency of navigating and documenting within an electronic medical record - Knowledge and proficiency of basic Word, Outlook and Excel skills - Previous experience and proficiency in ambassador role strongly preferred In order to work with potentially vulnerable patient populations and to keep employees healthy while caring for patients, all patient-facing employees of Landmark must provide evidence of the following required immunizations/vaccinations as a condition of employment:

The check will bounce and the victim is left responsible.
